There's no need to migrate the configuration to MS SQL. Only when there are changes, the database is accessed by the service. However, if you prefer to have the configuration database in MS SQL, you can do so.

No, you never need to restart the service after making changes to the configuration. There's only one exception: when making changes to one of the performance registry entries, trace files and advanced channel configurations you must restart the service.
